Requirements
Minimum of 2 years of experience in health and social care
Full UK driving licence with own transport
Responsibilities
Auditing medication records
Ordering medication from GP surgeries and mental health teams
Collecting medication from pharmacies
Delivering medication to our residents
Conducting medication reviews and risk assessments
Ensuring medication is available upon discharge
Updating medication records
Additionally, You Will:
Ensure the people we support receive high-quality medication support
Perform field medication spot checks
Build and maintain professional relationships with social workers, mental health teams, nurses, doctors, hospital discharge teams, speech & language therapists, and Wigan Council compliance team
Provide professional support to our carers, the individuals we support, and their loved ones
